DNP-A Application Instructions

We are glad you are considering the TCU-School of Nurse Anesthesia, DNP-Anesthesia program. Below is the checklist for all information that must be mailed in and/or completed online by all applicants. We suggest you print this page to insure all materials are received.

Note: Incomplete applications will not be reviewed. All applications and required materials, including letters of recommendation and application fee, must be received by July 1 st for class beginning the next January. Candidates will be notified of admission status by October 15th.

Checklist for Applicants

The following materials are handled online:

  • Application for Admission
  • Application fee: $60 (non refundable)
  • Personal Essay
  • Three Letters of Recommendation

The following materials are mailed in:

  • Two Official Copies of all College Transcripts
  • Clinical Experience Verification Form
  • Anesthesia Shadowing Experience Form
  • One copy of each of the following items:
    • Nursing license
    • PALS certification
    • ACLS certification
    • CCRN certification
    • BCLS certification
  • Mail materials to:

    Fort Worth, TX 76129

These Items Will Be Automatically Directed To TCU By Using The Following Institutional Codes When Taking The Exam

  •   Official GRE score report (code 6820)
  •   Official TOEFL score report, if applicable
